Case Manager provides many services such as:
Makes sure your loved one has their medications
separated by day and dose so they can take them at the correct
individual times themselves.
Calls the pharmacy and renews their medication when
needed.
Makes emergency visits if you call her/him to see your parent when you
cannot be in town to check on them.Supplies accurate information on
your parent or loved one.
Provides temporary assistance in your loved one's home for a short
period if they have some minor out-patient surgery, like cataract
surgery or periodontal surgery, just so they can have help there to make
sure they are eating, have their pain medication in reach, and can
answer the phone when you call.
Supervises the private duty Aide that is in your loved one's home |
